Insurance

For most commercial insurance plans we would be considered an out-of-network (OON) physician. Our office does not participate with any Insurance Plans. PPO and Indemnity plans may reimburse, at least partially, for our services, but you may be subject to copays, deductibles and co-insurance costs.  Full payment for our professionally fees is expected at or prior to the time of service.

All blood work or imaging (X-ray, CT, ultrasound, MRI) and Medications will be processed through your medical insurance plan as usual.  If you do not have insurance, blood tests can be provided at a significant discount via physician-only negotiated prices.  Our practice operates on a yearly subscription model that covers up to 10 visits (in-person or telehealth) yearly and direct access to the physicians in between. Our office accepts Cash, Zelle, Venmo and major credit cards.  Payments with credit cards are subject to a 4% convenience fee.

A receipt, a.k.a “superbill”, will be generated at the end of each visit and the patients can submit it with all necessary documentation to their respective medical insurance company for reimbursement according to the insurances’ out-of-network (OON) benefits policy. The office is not responsible for the amount of reimbursement from your insurance company.
